Position Overview:
Tarbiyyah Horizons is seeking a compassionate and skilled Case Manager & Support Services professional to join our team. The primary responsibility of this role is to develop and manage individualized care plans for children and youth in our care, ensuring that their mental, emotional, and cultural needs are met. The ideal candidate will be passionate about providing trauma-informed, culturally sensitive support to children from diverse backgrounds, particularly those from Muslim and Black communities.
Key Responsibilities:
Individualized Care Planning: Develop comprehensive and personalized care plans for each child, considering their mental health, emotional well-being, cultural identity, and individual circumstances. Ensure that these care plans are holistic, addressing physical, emotional, and social needs.
Culturally Sensitive Support:
Ensure that all services provided are culturally sensitive and in line with the child’s background and identity. Work closely with children, foster parents, and staff to promote an environment that respects and nurtures the child’s cultural, religious, and ethnic identity.
Case Management:
Oversee the day-to-day case management for children and youth, coordinating services such as counseling, educational support, health care, and family visits. Work collaboratively with foster parents, educators, and other service providers to implement the care plans and address the needs of each child.
Trauma-Informed Care:
Provide trauma-informed care and support, recognizing the emotional and psychological impact that adverse childhood experiences may have on the children. Implement strategies that promote healing, resilience, and positive emotional development.
Monitoring and Evaluation:
Continuously assess the progress of each child, updating care plans as needed to reflect changes in their circumstances or needs. Regularly review the effectiveness of support services and adjust them to ensure the best outcomes for each child.
Collaboration with Multidisciplinary Teams:
Work closely with other professionals, including therapists, social workers, educators, and medical staff, to ensure a collaborative approach to the care and support of the child. Act as a liaison between the child, their foster family, and external support systems to ensure that services are coordinated effectively.
Crisis Intervention and Support:
Be available to provide crisis intervention and emotional support when needed, ensuring that children and youth have access to immediate care during challenging times.
Family and Community Engagement:
Engage with families and communities to maintain and strengthen connections that are important to the child’s cultural identity and emotional well-being. Facilitate family involvement when appropriate, and work towards reunification or supportive family engagement.
